1. Application form

To apply for a Stolen Generation Reparations Package, please fill out the application form:

2. Nomination of a support person form

You can give permission for a friend, family member or support worker to help you apply for the package. This person is called a nominee. If you would like to do this, please fill out this form:

3. Application for an advance payment form

If you are applying to the package and are terminally or critically ill, you may be eligible for an advance payment. To apply, please fill out this form:

4. Request for a review form

You (or your nominated support person or organisation) can ask for a review if you do not agree with your outcome. To ask for a review, please submit a Request for a Review form:
